SAFE Transitional and Supervised Independent Living seeks a full-time case manager. SAFES Transitional Living Program serves youth ages 16-21 in a residential setting as they prepare and build skills towards independence. Our supervised Independent Living setting serves youth 18-21 in a non-college dormitory setting as they fine tune skills towards independence. The Case Managers role is to ensure that all services are provided in accordance with SAFE policies and procedures, current contractual, federal, state and local requirements, and Residential Childcare Licensing (RCCL) standards.

About this position:

  • We need you to fill the role of Case Manager of Transitional and Supervised Independent Living.  This is a full time, exempt position at our RV Campus location. We will pay you an annual salary of 35,000-37,000.

 

Key Duties:

  • Assess and convey the youth’s needs, strengths, and preferences via Service Plans and other forms of documentation. Recognize and value differences in cultural, religious, racial and ethnic backgrounds of youth, keeping in mind how they impact service planning needs.
  • Support youth in achieving identified goals geared towards independence to include prompt linkage to both formal and informal supports.
  • Instill trust through supportive relationships with youth and their teams while demonstrating proper boundaries.
  • Communicate in a timely manner serious incidents, concerns, and relevant information to medical and mental health professionals.
  • Ensure accurate and timely documentation of all case management services provided to youth.
  • For each assigned resident, meet together on both a regular and as-needed basis; in a resident-focused manner.
  • Demonstrate resilience and confidence under pressure, and show the ability to provide crisis intervention with youth as needed, including verbal and physical de-escalation in instances of a youth’s imminent risk of harm to self and/or others.
  • Exercise exceptional judgement and decision quality, especially when assessing risk, and develop and communicate realistic, safety and support plans for youth who are presenting with unsafe behavioral concerns.
  • Attend court hearings and/or any other interdisciplinary team meetings necessary for service planning and/or advocacy on behalf of youth.
  • Provide formal and informal training and consultations with direct care staff related to individual areas of expertise and be available to staff case management needs across campus and the agency.
  • Demonstrate resourcefulness and creativity when linking youth to potentially beneficial community services and supports.

 

Qualifications:

  • Graduation from an accredited 4-year College or University with a degree in social service related field required; graduation from an accredited college or university with a Master’s degree in Social Work or related field preferred.
  • Experience in a residential childcare setting, with work with teens and young adults, preferred.
  • Pass all required criminal history background checks (including an FBI fingerprint check), as well a pre-employment drug screen and TB test.
  • Be 21 years of age or older, with:
    • Valid, State of Texas Driver’s License;
    • At least three (3) consecutive years of driving experience OR one (1) year of driving experience if over 27 years of age; and
    • An acceptable driving record; and
    • If in possession of an out-of-state license, obtain a State of Texas Driver’s License within 90 days of beginning employment.
